This form is the official field trip permission form used at St. Gertrude School. Parents of students who attend activities away from St. Gertrude School are required to complete this form for each activity. We must have an original, signed form; a faxed copy of this form is not acceptable permission.

This form is used to pre-register for enrollment in St. Gertrude School. The 2008-2009 registration form is being revised. When it is ready, we will publish it, and this link will allow you to see it. The Pre-registration Form is also published annually in the St. Gertrude Church Bulletin during Catholic Schools Week in January and during the month of February.

This form is used with St. Gertrude School's Accelerated Reader Program.

In order to assure the proper use of Internet accounts and resources, it is necessary that all users and their parents sign this agreement before the user is allowed to access the Internet at school.
